What the form covers
The form includes declarations about:
Fit and proper status: confirming the landlord has no relevant criminal convictions, housing-related offences, civil penalties, or previous licence refusals or revocations.
Property compliance: confirming the property meets safety requirements such as valid EICR, gas safety (if applicable), working smoke/CO alarms, and correct waste and recycling guidance for tenants.
Identity verification: confirming the landlord’s identity with photo ID and proof of address.
Management arrangements: confirming who manages the property and that all information provided in the application is accurate.
Who needs to complete it?
UK-based landlords: must complete and sign the Landlord Declaration Form.
Overseas landlords: must instead complete the Overseas Licence Holder Consent Form, authorising their UK managing agent to act on their behalf.
